Embed the trial form on your website
Choose your platform for a step-by-step guide:
If you have your own website (e.g. self-hosted or with a hosting provider), add these two lines directly to your HTML code - either in the template or on the desired page:

The <script> tag can go in the <head> section or before the closing </body> tag. The <div> goes wherever you want the form to appear on the page.
Enter your club subdomain in the sidebar (the same one you use to log in to Omoplata) to automatically update all code examples on this page.

Dark and light theme
The widget comes in two themes to match your website's background. Set the theme using the data-theme attribute:
- data-theme="dark" - dark background, ideal for light-colored websites.
- data-theme="white" - white background, ideal for dark-colored websites.

How it works
When a visitor fills in the embedded form and submits it:
- A new lead is created automatically in your Omoplata leads pipeline.
- If your club has a trial contract configured, the visitor must accept the terms before submitting. A signed PDF is generated and attached to the lead record.
- The lead appears in the New column on your Kanban board, ready for you to follow up.
The entire flow - from form submission to lead creation - happens in real time. No manual data entry required.
Time slot selection
Optionally, you can enable time slot selection so leads can pick a specific class for their trial directly from the form. When they do, they receive an email with a calendar invitation - this creates a real commitment and significantly reduces no-show rates. You can enable this in your trial settings.
Tips for a smooth integration
- Test the form on your site before going live. Embed the widget on a staging or hidden page first to verify it loads correctly and matches your design.
- Choose the right theme for your background. Use data-theme="white" on dark pages and data-theme="dark" on light pages so the form fits naturally.
- Place the form where it is easy to find. Common placements include the homepage, a dedicated "Free Trial" page, or alongside your class schedule.
- Configure your trial contract first. Before embedding the form, make sure your trial contract is set up in Settings > Legal. This ensures every lead signs the terms automatically.
- Share your subdomain with your web developer. If someone else manages your website, they only need your club subdomain and the two lines of HTML above.
